Teen
Court
that has shown progress, innovation, and/or achievement. Please include the

on a separate sheet of paper and submit with your registration form.
Include Name, Address, Coordinator’s
name, Sponsoring agency, Phone, fax and Email. Also complete sections as follows:
Section I: Describe how your court functions and
include such items as your annual caseload, sentence completion rate and methods
used to dispose of cases.
Section II: Describe what you consider to be unique

of other agencies and organizations with the community.
